Create a New eForm

The below instructions cover each of the options available when building a form.

The article is long, although if comfortable with the options, focus on the steps, not what is contained in the tables. Use the table as a reference to unknown field.  

It is highly recommended to continually click Save Changes during the creation of an eForm.

  1. Within the eServices portal:

    1. Click hamburger menu > e-Services  > eServices Management
    2. Select eForms Manager

  2. Select Location (if applicable)

    1. Click Create Form

    2. Click the Edit icon > Enter Name of eForm

    3. Click Save

  3. Options:
    1. Open Design Theme and select the relevant Colours, Font and Header 
       Colors   Select Background, Labels and Headings, Main Font colours 
       Font   Select the Font style and Font size 
       Header   Select Title name or Upload logo and use the editing features to make change 
    2. Open Add Field and click on the field(s) to add, and then drag into place on the form (right)
       Heading  Enter the name of the form. eg. Medical History Form
       Select the font size
       Align the text
       Add to a group (see manage group section of this doc)
       Text/Number   Create a name for the Label field – e.g. Emergency Contact Number
       Tick Required box if this field is compulsory
       Does the field require characters / numbers or both
       Field size
       Max number of characters/symbols allowed
       Use pattern is available when 'Any Symbols' is selected, provides the ability to dictate what  combination of characters are entered
       (e.g. 5 numbers followed by a letter would be a pattern of #####~)
       Add to Group: See 'Manage Groups' section of this document

       Text Area 

       Create a name for the Label field – e.g. Occupation
       Tick Required box if this field is compulsory
       Field size
       Select how many rows the text area should be in the Height field
       Enter the maximum number of symbols allowed (if any) in the Input Limit field
       Add to Group: See 'Manage Groups' section of this document

       Paragraph

       Create a name for the Label field – e.g. Disclaimer
       Select the size of this text
       Align the text
       Add to Group: See 'Manage Groups' section of this document

       Dropdown 

       Create a name for the Label field – e.g. Preferred Method of Contact
       Tick Required box if this field is compulsory
       Field size
       Add to Group: See 'Manage Groups' section of this document

       Options: Enter the value – e.g. Phone, Show groups,
       When the form is completed by a patient the selected form will be opened automatically 

       Radiobutton 

       Create a name for the Label field – These are generally questions.
       E.g. Are you happy with your smile?
       Tick Required box if this field is compulsory
       Add to Group: See 'Manage Groups' section of this document

       Options: Enter the value – e.g. yes, Show groups,
       When the form is completed by a patient the selected form will be opened automatically
       Tick 'other Option' if you would like an 'Other' checkbox onscreen with tick box
       Tick 'Sort Alphabetically' if required
       Select how many columns to spread the answers / buttons across

       Checkbox Create a name for the Label field – E.g. Preferred Method for Appointment Reminders?
      Tick Required box if this field is compulsory
      Add to Group: See 'Manage Groups' section of this document
      D4W Field (Internal use only)
      Options: Enter the value – e.g. SMS, is Y/N Answer box required, Show groups, When the form is completed by a patient the selected form will be opened automatically
      Tick 'other Option' if you would like an 'Other' checkbox onscreen with tick box
      Tick 'Sort Alphabetically' if required
      Select how many columns to spread the answers / buttons across
       Date  Create a name for the Label field – E.g. Last time you visited the dentist?
       Tick Required box if this field is compulsory
       Add to Group: See 'Manage Groups' section of this document

       Time   Create a name for the Label field – e.g. What time do you usually fall asleep?
       Tick Required box if this field is compulsory
       Add to Group: See 'Manage Groups' section of this document

       Email  Enter / Edit name of Email field
       Tick Required box if this field is compulsory
       Add to Group: See 'Manage Groups' section of this document

       Signature  Invitation text has a Default value: Patient Signature
       Tick Required box if this field is compulsory
       Select the size of the field – look at the preview on right to check

    3. Open Add D4W Field and select the fields from D4Web to add to the form, drag and drop into place on the form.
      1. If using Custom Fields in D4Web, they will be listed here
    4. Manage Groups is only used when a dynamic question should be added to the form, for example when asked if the patient is pregnant, the form will ask for more details
  4. Click Preview to view the form as a patient will mceclip3.png

  6. Click Form Manager
