National Dental Plan: Submit an Application


The patients National Dental Plan application will start from the Treatment Plan page.

The patient can only have one application per Treatment Plan.

Submit Certegy Application

  1. Click the navigation menu > Patients > Treatment Plan page.

  2. Treatment Plan toolbar > NDP Application

    1. Ensure the correct Provider is selected as these details are used during the application.
    2. Select items to include in the application
    3. Select the Responsible Party – the person responsible for payment
    4. The Deposit amount auto-calculates based on the minimum 20% deposit. 
    5. The Repayments will auto-calculate the fortnightly payments
    6. Click SUBMIT APPLICATION buttonmceclip0.png
    7. A passcode will show onscreen. Note this down as it will be requested upon application.
  3. Open the National Dental Plan portal:

    1. Read and agree to the Terms and Conditions

    2. Enter the passcode generated by D4Web

    3. Click Verify my Access Code button - The application can be opened on any device with a browser, but the Practice must fill in and verify details of the patient.

  4. Fill in the blanks on the application form and press Next. Some details pre-populate based on fields in D4Web
  5. When the below window appears, the patient/applicant will receive an SMS and must follow the steps to digitally accept the application.

    1. The tick boxes indicated below will update as the patient/applicant progresses through the steps

    2. Advise the patient/applicant to open the Agreement link
    3. Provide the patient with the Unlock Code shown onscreen
  6. Once the patient accepts the application, the approval will show onscreen.

  7. I confirm the customer has received the goods/services:

    1. Click Yes or Nomceclip3.png

    2. Click Redirect Now to exit the application

  8. The Merchant/Practice will receive an e-mail with the Agreement and the patient will receive:

    1. An SMS containing a link to download the Agreement

    2. (If e-mail supplied during application) an e-mail copy of the Agreement

Patient SMS and Application Steps.

Once the application has been submitted, the patient will receive an SMS with instructions to agree to the contract.  

  1. The 3rd link found in the SMS is the Agreement

  2. Applicants should read details within other links also

  3.  The patient enters Unlock Code supplied by the Practice

  4. The patient accepts the application

  5. If the Patient/Applicant selects Exit or I do not agree, the Merchant can print the agreement and have them sign.
